Add Signature Forms to Intake Forms

  • Go to All Tabs (+ sign)



  • Go to Online Configuration tab



  • Scroll to the Intake Form Configuration Center section



  • Fill in the desired form title and text - You can copy and paste form text from your current PDFs or Word documents.



  • After filling out all the forms, scroll to the end of the Intake Form Configuration Center and click "Save Intake Form Config" button. (Save frequently so the browser doesn't timeout on you!)
